Forms to complete

On this page you will find links for various forms that can be downloaded and completed to support your event application. Forms include the Event Notification Form for the Safety Advisory Group (SAG) and the Yorkshire Ambulance Service (YAS) Medical Assessment Form for determining your First Aid and Medical provision. The page also includes links to booking forms for the hiring of land, Licensing application forms in addition to examples and templates for a Risk Assessment and an Event Management Plan that can be adapted to fit your event.

The links below are examples and templates for a Risk Assessment and an Event Management Plan that, depending on the size and scale of your event, can be adapted for your event. Other examples are available via the internet.

The documents should be completed and submitted to the land owner and SAG (sag@bradford.gov.uk) to support your event application. Remember to also send a site plan showing your event layout and evidence of public liability insurance.

Booking forms for the hiring of land

Forms for completion and submission to the relevant departments. Permission to use the land from the land owner should be sought before you pursue the booking of entertainment and infrastructure etc.
